You will start to prepare for leaving care at about the age of 15. As part of the planning process, you will be allocated a social worker from the 16+ team who will visit you to discuss your future.
Your social worker will carry out a Needs Assessment. You and your social worker can create a full picture of what you need and what you want to happen.
Leaving care is an important time in your life and you will have the freedom to choose many things for yourself. The more information you put into your Needs Assessment, the more likely you are to be satisfied with the arrangements after you leave care.
Using the information in your assessment, you and your social worker will start to create your very own Pathway Plan.
