At your first review you will be given a health record which will help you to keep a record of things such as injections or illnesses you may have had before you came into care. Social Services need this information to help keep you healthy while you are looked after. Whenever you have an appointment at the Doctors or Dentist, these will be written down in your health record. You can then keep track of your appointments so that when you are no longer in care you will still have a record. Your carer and your social worker will make sure that if you are ill, you will be given help to make you better. There are some tips on this website for things you could do to try and keep yourself healthy.
You will also be asked to have a general health assessment each year. This is to make sure that you are healthy and so that you get the chance to ask your doctor any questions you may have about your health. If you don't want to see the doctor, you could ask your social worker if you can see the Designated Nurse for Looked After Children (LAC Nurse) instead.
You don't have to go to this alone if you dont want to. You can ask your social worker, foster carer or someone else you trust to come with you for some support.

Who Are The Looked After Children Nurses?
They are Emma and Linda - it is their job to make sure that you, your carers and your social worker have the information you need to keep you healthy. You can call Emma or Linda on 020 8545 4246 or you can email them at lac.nurse@merton.gov.uk.
You can call to ask for confidential advice and if they are not there you can leave a message and either Linda or Emma will call you back as soon as they can.
What does a school nurse do?
The school nurse helps your school to make sure the pupils are healthy. School nurses aren't at school all the time but they do come in to give advice and to help the pupils who have any problems or issues about health. Your school office they will be able to tell you who your school nurse and he/she will be visiting your school.
