What is a care plan and who puts it together?

Social Services must make plans for all of the children and young people they are looking after. These are called Care Plans. Your care plan will cover:

• Where you go to school/college;
• when you see your family;
• what you want now; and
• what you want to do in the future - whether or not you want to be a astronaut or the lead singer in the next big band!

Your social worker will put together your care plan after talking to you and others - foster carers, family and maybe even teachers.   If everyone is happy with it, it will be agreed at the Review Meeting.

Just like you, your care plan may change over time. This is to make sure that your changing needs are met and we review your plan regularly.
