Your social worker will make plans for you, so that you have all the things that you need whilst you are looked after.
Your social worker will come and visit you at least every 6 weeks and if you are looked after for more than 12 months, then your social worker will visit you at least every 3 months to make sure that you are getting on okay. You can talk to your social worker about how you are and you can discuss the plans that are being made for you.
If you are having any problems, are feeling a little bit worried about something, or just fancy a chat, your social worker will try and help you.
Above all, your social worker will work with you and your family to try and make it possible for you to go home.
